Three pull requests merged into projects I do not own. I am not a maintainer and not a regular contributor: I am a contributor. Each entry below has the problem, the fix and the review, with a link to the public PR if you want to read the diff.

vitest-dev/vitest

PR #11360

Merged on September 29, 2026

The problem

The vm pools ran Vite's dependency optimizer over the whole index.html. If a dependency discovered that way could not be bundled, the run died with exit 1 while every test passed. That is the worst kind of failure: it does not say a test is wrong, it says the process crashed.

The fix

The configEnvironment handler returned early for the vm environment before applying resolveOptimizerConfig, so that environment kept Vite's browser default. I traced it back to the commit that introduced it and the fix was merged. The maintainer added wording nits and clarified the fixture before approving.

Verification and review

RED → GREEN: without the fix exactly vmThreads and vmForks fail; with the fix all four pools pass.

fastify/fastify-cookie

PR #377

Merged on October 27, 2026

301 stars

The problem

reply.setCookie() accepted an encode option and ignored it: it never forwarded it to the cookie@2 serializer, so there was no way to customise how the value is encoded. A silent failure: the option goes in, nothing happens.

The fix

I separated encode from the rest of the attributes and passed it as stringifySetCookie options. It was merged with two maintainer approvals and shipped in @fastify/cookie 11.1.5, minutes after the merge.

Verification and review

An earlier PR had the same goal. Instead of closing mine, I explained how it differed; the maintainers merged both and the code that survived is mine. The PR took 32.6 days to land.

vitest-dev/vitest

PR #11781

Merged on October 30, 2026

17,200 stars

The problem

Since 5.0.4 the on-disk module cache is enabled by default. If its directory could not be created (a read-only filesystem, or a project owned by another user), the mkdirSync threw, and the module being transformed failed with an error that had nothing to do with the test.

The fix

I measured that the failure hit the first module of each config, not every file as the report claimed. Instead of propagating the error, the cache degrades: it reports the directory as unusable once and returns the same null value that already meant "this module is not cached".

Verification and review

The maintainer requested changes: he wanted the whole feature disabled so no hashes were computed for nothing. My first version caught the error where the mkdir lived, which is too late. I moved the check to the start of the traversal: it does not read, does not hash, does not write. The final design is smaller than mine and better.

Why you can verify this

Every card links the public pull request on GitHub: the diff, who approved it, who merged it, and what was discussed in review are all there. I do not list open or rejected PRs, because an open PR is an intention and can still close without being merged.
